  4. 4Review findings and receive an individualized plan

We use intake forms that match your situation. New patients are not required to complete extensive functional medicine questionnaires up front.

MVP framework, when relevant

The MVP Diagnostic Model may inform care when the clinical picture is more complicated, but it is not presented as mandatory for every new patient.

Referrals when appropriate

If your situation calls for another specialist or level of care, we will say so and help direct you.
