How long should concussion symptoms last?

Short answer

Most concussion symptoms improve substantially within a few weeks. When symptoms persist beyond about four weeks, targeted evaluation of the systems involved is generally recommended rather than continued waiting.

Recovery time varies with the injury, prior history, age, and how care is managed in the first days and weeks. Prolonged strict rest is no longer the standard of care; early, structured, sub-symptom activity supports faster recovery for most patients.

When symptoms persist, it is usually because one or more specific systems have not recovered — cervical, vestibular, oculomotor, autonomic, cognitive, or metabolic. Identifying which system is driving the persistence lets treatment target it directly.

When evaluation may be appropriate

Symptoms lasting beyond several weeks, worsening, or interfering with work, school, or driving should prompt a formal post-concussion evaluation.
