What should I bring to the first visit?

Short answer

Bring your ID and insurance information, a list of medications and supplements, and any prior records — imaging, reports, testing, or provider notes — that relate to your case.

You don't need to have prior records to be evaluated. If they are available, they help the evaluation move faster and reduce duplication. If they are not, we can request them or determine what is genuinely needed based on findings.
